General anesthesia with remifentanil for cesarean section in a parturient with severe mitral and tricuspid regurgitation

Authors

Abstract:

Valvular heart lesions have deleterious effects on hemodynamics in parturients during pregnancy. Cesarean section with an opioid-based general anesthesia is used to alleviate the adverse effects. We described a case of a 28-year-old primigravida at 37 weeks’ gestation with a diagnosis of severe mitral regurgitation, tricuspid regurgitation, and pulmonary hypertension in active labor. Cesarean section under general anesthesia using remifentanil was planned to provide a stable anesthesia and analgesia to minimize hemodynamic changes during surgery. The patient maintained reasonable hemodynamics during surgery. The neonate required transient ventilatory support with bag and mask until she obtained spontaneous respiration. We suggest that remifentanil is a reliable opioid that can produce stable hemodynamic conditions with transient neonatal respiratory depression during cesarean section under general anesthesia in a parturient with severe valvular heart disease and pulmonary hypertension.

The use of remifentanil in general anesthesia for cesarean section in a parturient with severe mitral stenosis and pulmonary edema.

Valvular heart diseases have adverse effects on hemodynamic condition in the parturients during pregnancy. Cesarean section with an opioid based general anesthesia has been used to alleviate these deleterious effects. We hereby describe the effective application of remifentanil, for cesarean section under general anesthesia, in a 30 yr. old primigravida suffering of severe multivalvular heart d...

Remifentanil at induction of general anesthesia for cesarean section

INTRODUCTION Remifentanil, with its rapid activity onset and short duration of action, may be more effective than other opioids for providing hemodynamic stability during obstetric anesthesia. However, there is some evidence of adverse effects on neonatal respiratory function. We investigated maternal and fetal effects of Remifentanil during cesarean section surgery. METHODS Eighteen women wi...

Anesthesia with etomidate and remifentanil for cesarean section in a patient with severe peripartum cardiomyopathy--a case report.

Patients with peripartum cardiomyopathy may require analgesia/anesthesia for delivery or cesarean section. Many different methods of anesthesia has been used for this purpose. Remifentanil was used safely in peripartum cardiomyopathic patients, but there is not any report about etomidate usage in such patients. We report on a 19 years old patient, at 32 weeks of gestation, with severe peripartu...

Clinical case discussion: anesthesia for Cesarean section in a parturient with a large intrathoracic tumour.

PURPOSE To report the anesthetic management of a parturient with a large intrathoracic tumour, presenting for Cesarean section. CLINICAL FEATURES A 28-yr-old parturient, gravida 1, presented at 33 weeks gestation with a one month history of increasing cough and dyspnea. A computed tomography scan demonstrated a large mass filling the right hemothorax, causing mediastinal displacement to the l...
